How does the program work?Our application and funding process is quick and easy. Once a short patient credit application is submitted, you will receive a credit decision in as little as 20 seconds. We then provide you with a Purchase Verification Invoice (PVI), a one-page form detailing the dollar amount of the credit line with all credit approvals. On the day the treatment plan is started, simply complete the PVI and submit it online or fax it to a toll-free number for non-recourse payment. We will process payment to your practice within about 24 hours via ACH (direct deposit).

What are the fees to join and use ChaseHealthAdvance services?None. There are no sign-up fees or monthly maintenance fees. There are no additional fees to use our service online to apply for credit, track credit decisions, funding status, and open lines of credit.

How do patients apply?For your convenience, we offer multiple options. Patients can apply right from your office by filling out a short application that can be faxed to our office. Your staff can complete an even shorter application online and receive a response in seconds. At home, your patients can even submit applications online or over the phone.

How do I get paid?Complete an electronic invoice from our secure Web site and submit it to us. There's no faxing or complicated paperwork necessary. We will process your non-recourse payment within 24 to 48 hours by ACH (direct deposit).

Why is there a service fee charged to the practice?We charge a fee to help cover our lending risk. Payment is non-recourse.

How can I keep track of my activity?You will have access to HealthAdvance-Online, a secure Web site that provides daily reports of all activity, including credit decisions and funding status, plus a Pipeline Report of patients who have not used available lines of credit to pay for their treatment plans.
