Chase Bank is phasing out the patient financing loan product known as ChaseHealthAdvance. If you would like to apply for a ChaseHealthAdvance Account, please be aware that we are only working with a select group of providers at this time.

If you apply and your application is approved, you must use your Account on or before April 20, 2012. Chase will no longer be accepting applications or purchase transactions after that date. Contact the provider you select for additional details.
